ONLINE PRESCRIPTION ORDER FORM FOR COLLECTION FROM THE PRACTICE

Please ensure all fields are completed to allow us to process your prescription accurately.

Online prescription requests placed between Monday and Thursday will be dispensed ready for collection from the practice the following afternoon. Orders placed from Friday morning through to Monday will be available for collection from Tuesday afternoon.

Please provide a relevant phone number on which we can call you if we have any questions regarding your pet’s prescription. In certain circumstances it may be requested that we examine the patient before dispensing more medication.
